Prescription Benefits Change

Our opportunities and choices continue to grow! The most recent Company benefit is the Employee Stock Purchase Plan. This program allows full-time employees to purchase stock in Quanta Services, Inc. at a 15% discount through payroll deduction. If you are not already participating in the program and would like more information on it, please let me know. You can enroll at either of the two offering periods each year (January 1 and July 1). You can change your deduction percentage at those times, also. The only other time you can change your percentage is at the mid-point of either offering period. Changes can be made with another enrollment form or other written notice prior to the appropriate date.

You can change your deduction percentage on your 401(k) retirement plan the first of each quarter (January 1, April 1, July 1, and October 1).

Effective December 1, 1999, the prescription claims processor for our Blue Shield of California Health Plan changed to ARGUS Health Systems. Participating pharmacies have been informed of this change.

Member co-payment for prescription drugs continues to be $10.00 for generic and $15.00 for name brand drugs. Mail-in prescriptions are now serviced by Express Scripts, Inc. The member co-pay for a 3 month supply of a generic drug is $20.00 and $30.00 for name brand drugs.

If you would like more information on the mail in prescription services, please just give me a call. Mail in prescriptions can be a real convenience, as well as a cost savings, for maintenance medications.

When you’re in the Sacramento area, listen to the local radio stations for a few minutes, and you’ll probably hear an advertisement for Roseville Telephone Company (RTC). They’re proud to offer services ranging from wireless to digital — available with just a phone call.

RTC is a forward-looking company with a vision of how the community is changing and how they can best serve their customers. In order to provide expanded services, RTC has moved rapidly into the high tech digital field. The installation of Controlled Environmental Vaults (CEV’s) and fiber optics has certainly posed no

problem in developing their plan.

Roseville is a growing community. The Del Webb communities, along with various other housing tracts, are popping up everywhere. With new housing comes new shopping centers and new businesses, and RTC will be there to provide the necessary telephone facilities.

Our long association with RTC has spanned almost 30 years. We’ve watched them grow, and we strive to keep up with their progress; no small goal when you consider the aggressive schedule we maintain while digging through less than ideal soil conditions (lava rock). We recently labored for several days, and expended numerous backhoe teeth, in order to place a 35’x15’x14' CEV. The excavation and end result were quite impressive.

During our association, we have had the privilege of working with some wonderful people such as Gene Cornthwaite, the area manager, and Donna Fretwell, his right-hand

assistant. Bob MacDonald has office space in RTC’s beautiful facility, and Tom MacDonald and David Gross run our crews out of the Roseville yard. Our working relationship has allowed us to feel that RTC facilities are “our facilities” and we take pride in doing a quality job for them. Quality, however, can only be achieved through a safe work environment, and RTC has always supported our safety goals 100%.

It’s a pleasure to work with a company that appreciates our efforts. Roseville Telephone Company is a model “Owner” to work for; their management and supporting staff are top rate. Their assistance and support makes our job much easier. So listen and watch for RTC’s advertisements, and you’ll understand why we continue to stay so busy with our partners in Roseville.
