Benefit Reminders *Don't forget that you can change your 401(k) percentage contributions at the start of each quarter (Jan. 1, April 1, July 1, and Oct. 1). *You may change your deductions for the Employee Stock Purchase Plan (or enroll for the first time) 2 times each year - Jan. 1 or July 1. Once you are participating in the stock purchase, you may also change your deduction percentage one time during the offering period. (This is at the mid-point of each offering period). *Remember that you must enroll a new dependent in your medical and dental insurance within 31 days of acquiring that dependent. It's a hectic time when you bring that new baby home, but don't forget! Your initial enrollment in benefits must be within 31 days of your becoming eligible, but it will make things easier for you to get the forms turned in prior to the date you are first eligible. Please be sure that we have your current address on file. Important information about your benefits may not reach you if we do not have the correct address! *If you receive a bill for medical or dental services that you don't understand, first call Member Services at (800)-424-6521 for medical and (800)-325-6541 for dental.
